Scholarship & Grant Opportunities
SSM Health St. Clare Hospital - Baraboo Foundation offers Healthy Community Partnership grants which provide funding for our projects, programs and activities that advance the health and wellness of those living in the greater Baraboo-Lake Delton-Wisconsin Dells area.
Those working on innovative ideas, collaborative projects or programs who need seed money to implement the project are encouraged to apply. The maximum grant award is $2,000 and the grants are awarded in two semi-annual cycles. The application deadline for the grants are April 15 and Oct. 15, with all funds awarded by June 15 and Dec. 15.
The SSM Health St. Clare Foundation - Baraboo's Healthy Community Partnership grant initiative began in 2007 and is funded by a portion of investment income earned from the Foundation's endowment. The endowment also funds medical equipment for SSM Health St. Clare Hospital - Baraboo facilities, as well as local health care programs and services. To date, the Foundation has awarded $88,000 in Healthy Community Partnership Grants to community organizations and groups to promote well-being across the age spectrum.
Eligible programs and activities for funding include, but are not limited to, health education, disease screening, fitness and wellness assessment and improvement, research, speakers' fees related specifically to health and wellness and serving the area's underserved. The project must impact residents in SSM Health St. Clare's service area (the greater Baraboo-Lake and Delton-Wisconsin Dells area). Practitioners in all fields of health, educators and school districts, employers, civic organizations and service clubs are encouraged to apply. Collaborative proposals are always welcomed.
